The said vessel encountered very heavy weather during the voyage on the way between Lat 06ْ 34’N,Long 109ْ 10’E and Lat 15ْ 31’N, Long109ْ 44’E as per Logbook abstracts attached.
And fearing loss and/or damage, I hereby note my protest against all loss, damage, etc and reserving the right to extend the same at time and place convenient.

二.海事报告
1. 船舶,设施发生海上交通事故,必须用VHF 或其他有效手段向就近港口的港务监督报告。报告的内容包括:船舶或设施的名称,呼号,国籍,起讫
港,所有人或经营人名称,事故发生的时间,地点,海况以及损害情况,救助要求等;
2. 船舶,设施发生海上交通事故,除应立即提出初步的扼要报告外,还必须按下列规定向港务监督提交“海上交通事故报告书”和必要的文书资料:
1) 船舶,设施在港区水域内发生海上交通事故,必须在事故发生后24小时内向当地港务监督提交; PAGE 58
2) 船舶,设施在港区水域外发生海上交通事故,船舶必须在到达我国的第一个港口后48小时内向港务监督提交; 3) 引航员在引航船舶的过程中发生海上交通事故,应该在返港后24小时内向当地港务监督提交;
4) 中国籍船舶在我国沿海水域以外发生的海上交通事故,其所有人或经营人应当向 船籍港港务监督报告,并于事故发生之日起60日内提交“海上
交通事故报告书”;
5) 派往外国籍船舶任职的持有中华人民共和国船员职务证书的中国籍船员对海上交通事故的发生负有责任的,其派出单位应当在事故发生之日起60
日内向签发该职务证书的港务监督提交“海上交通事故报告书”
3.“海上交通事故报告书”应当如实写明下列情况:
1) 船舶,设施概况和主要性能数据;
2) 船舶,设施所有人或经营人的名称,地址; 3) 事故发生的时间,地点; 4) 事故发生时的气象和海况;
5) 事故发生的详细经过(碰撞事故应附相对运动示意图); 6) 损害情况(附船舶,设施受损部位简图); 7) 若船舶,设施发生沉没,指明沉没概位; 8) 与事故有关的其他情况。

Dear Sirs,
I hereby declare that the cargo had been discharged from my ship M.V . : “C H S MAGNIFICENCE” is 153,000MT till 0700hrs/19th and Balance is 8,022MT, but the shore figure is 151,100MT been discharged, and Balance is 9,922MT, So shortage in discharging, and the ship can’t accept the shortage at present and/or final , so if any problem about the matter, the ship, owner is not responsibility for any claim raised. Please note that we are monitoring discharge record per shore scale with ship’s draft survey every day. YOURS FAITHFULLY, ______________________________

二.海事报告
